Required Qualifications:

• Bachelor\'s Degree: From an accredited Occupational Therapy Program OR Master\'s Degree: From an accredited Occupational Therapy Program OR Ph.D: From an accredited Occupational Therapy Program.
• Within 30 days of hire: National Provider BLS - American Heart Association
• Upon hire: Oregon Occupational Therapist License
• Upon hire: Washington Occupational Therapist License - if practicing within this state
