Provides triage services in an outpatient clinic setting. Services are provided by anticipating needs, providing, and managing care, under general direction of physician team. Navigates the patient's care collaboratively across multiple disciplines, departments, outside agencies, and providers within the system to achieve seamless patient care. Takes a leadership role and collaborates with the multiple-disciplinary care team and other caregivers to meet specific goals.SCHEDULE: Monday - Friday 8a - 5SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:Must have an RN license in the state of Idaho issued prior to hire date.Multistate licenses must establish residency with the Idaho Board of Nursing within 60 days from hire.All colleagues must provide licensure or proof of application in process for an Oregon RN license within 90 days of the hire date.Bachelorxe2x80x99s degree in nursing strongly preferred.Minimum of 2 years of nursing experience preferred.Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hirESSENTIAL FUNCTIONS:Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions.Performs phone triage and post-discharge phone calls within 72 hours of discharge.Educates patient based on assessment of needs.Works as the patient's advocate necessitating a seamless continuum of care. Involves family/caretaker in plan of care and conducts patient and family education as appropriate. Explores the patient's understanding and knowledge of current health status.Communicates and coordinates the plan of care with the patient which includes discharge OPAT orders.Ensures patient has full access to the services and support resources as needed.Tracks time-sensitive labs for all OPAT patients and expedites results to the provider/care team as appropriate. May need to manually transcribe lab results into patient chart.Triages all antibiotic levels.Performs EKGxe2x80x99s and gives vaccines.Provides communication to the primary care provider about the patient's plan of care as needed.Collects and records data that is comprehensive, accurate and systematic.
