Primary Purpose:
Functions as the key linkage for homeless persons with mental illness in the Honolulu to Central Oahu areas. Acts as the primary liaison between the ACCESS line and the Community Based Case Management-Recovery Services (CBCM-RS) programs. Responds comprehensively to the needs of clients through outreach, referral, and case management.
Essential Duties:
Provides outreach contacts to at least 225 people on an annual basis.
Documents at least 750 face-to-face encounters on an annual basis.
Provides assistance, as needed, to Level II and Level III Outreach Workers by partnering with them, either individually or as a group, on all outreach activities.
Provides services to lower acuity clients with less complex needs.
Conducts outreach in areas frequented by homeless people, identifying and engaging target population with goal of referral to HCHP and AMHD services.
Assists clients in acquiring emergency supplies, including food, clothing, bedding, medicine and hygiene items.
Assists clients in applying for entitlements and obtaining identification. Where needed, takes clients through entitlement, medical, and mental health systems to connect them with services and benefits.
Assists clients with legal entanglements.
Conducts intake and assists in assessment, counseling, and referrals for health and social services.
Makes appointments for clients and escorts them to such appointments as needed.
Provides counseling, consultation, and crisis intervention as needed. Encourages medication adherence as appropriate.
Assists with family and social relationships as appropriate. Where needed, teaches communication and social skills development.
Provides interim case management services for each client which include:
- outreach / engagement / housing needs and home visits
- preparing a plan, based on clients\' needs and goals, for the provision of services
- meeting with clients on a regular basis
- following up with clients and providers of services to determine outcome of linkages
- providing psycho-social rehabilitation services (teaching hygiene and self-care, housing skills, budgeting, problem-solving, etc.)
- reviewing treatment plans at least every six months
Provides back-up and coverage for other outreach workers and case managers when necessary.
Advocates on behalf of clients to ensure equity of treatment.
Adheres to HCHP Outreach Guidelines.
Compiles quarterly/annual statistics and forwards them to the Project Director as required.
Keeps records of all client cases, and is responsible for ensuring that the records are entered into the clinic computer data-base, and for safeguarding clients\' confidentiality and other rights.
Works with other Project and KPHC staff in providing a coordinated system of care to the clients.
Works as part of a team with other staff for purposes of outreach, back-up, and shared service delivery.
Assists other Project staff in obtaining information on clients from other agencies.
Regular and reliable on-site attendance is required.

Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.
