home and facility-based palliative care visits and community resource navigation.

Job Overview



As a

Contracted Palliative Care Patient Navigator

, you will provide compassionate, patient-centered palliative care services in

home and facility settings

. You will work closely with an interdisciplinary team, reporting directly to a

Nurse Practitioner

, and play a vital role in supporting patients and families with technology support and helping to access necessary care, support and resources improving their quality of life.

This position is

per-visit based

, allowing flexibility in scheduling. It may lead to

full-time employment

as our company continues to grow.

Key Responsibilities



Conduct

home and facility-based palliative care visits for technology support

for patients with serious illness. Collaborate with an interdisciplinary team, including nurse practitioners, physicians, and social workers, to develop and implement individualized care plans. Act as a guide for patients navigating complex health issues while connecting them to community resources. Submit availability (days and hours) to help coordinate patient visits. Document all care activities in the electronic health record (EHR) in a

timely and accurate manner

. Maintain communication with the care team and participate in team meetings as needed. Coordinate care transitions with hospice or other providers when necessary.

Qualifications



High school diploma or equivalent required.

Previous experience

in healthcare or medical customer service, especially in a patient-facing role required. Strong organizational and problem solving capabilities. Ability to work independently while being an integral part of the palliative care team. Excellent interpersonal and communication skills. Must have a

reliable vehicle

, valid driver's license, and auto insurance. Proficiency with electronic health records (EHR) preferred.
