Description

CIFC Health, a project of Connecticut Institute For Communities, Inc., seeks full-time Pediatric LPN. The LPN works directly with clinicians, within the scope of LPN licensure, to facilitate quality patient communication and care, and efficient patient flow. The LPN may also assist in the preparation for procedures, recording of patient information, and clinical equipment maintenance.

JOB RESPONSIBILITIES:

  • The LPN\'s role and duties are designed to gather information to be used by the providers in assessment and patient care. The LPN may also assist in the handling of equipment in preparation for procedures, or follow-up to procedures, and equipment maintenance.
  • Provides patient/family education including education on after hours, triage and referral services. In addition, provides education to patients/family on applicable disease states.
  • Contacts specialists to obtain letters and results and helps to set up home therapies if appropriate as determined by a provider.
  • Acts as a patient advocate to help patients navigate the medical system as effortlessly as possible by advising patients on available services and appropriate follow-up.
  • Makes follow-up appointments with CIFC practice or specialist, educates patients/family on when an ER visit is appropriate and the importance of continuity of care.
  • Provides triage for calls on pediatric and adult patients, using established protocols to determine appropriate care, including at home management, or timing and location of medical visit.
  • Provides follow up on overdue tests and diagnostic images.
Within the scope of LPN License: * Assists with physical exams: completes required screenings and laboratory testing.
  • Administers treatments/immunizations/medications to patients as ordered by the physician.
  • Provides patient/family education including education on after hours, triage and referral services. In addition, provides education to patients/family on applicable disease states.
  • Completes and maintains all required documentation.
  • Coordinates services with/for the patient. Acts as a patient advocate to pursue financial assistance, initiates referrals to community based organizations as needed (Home Care, Healthy Start, Dental, and Behavorial Health).
  • Collaborates with school nurses, WIC colleagues, DCF and other medical departments as needed. Completes paperwork needed by external organization (e.g. school PE forms).
  • Contacts specialists to obtain consents, progress notes, and results and helps to set up home therapies if appropriate.
  • The LPN is not permitted to perform activities which require a license as a health care provider beyond the level of a Licensed Practical Nurse.
  • Assist the patients with prescription needs/refills.
  • Counsels patients regarding treatment, its side effects, preventative measures and make appropriate referrals.
  • Assures that appropriate forms, reports are accurately completed (school, Physical, DMV, WIC etc.) for clinician review and signature.
  • Acts as a patient advocate to help patients navigate the medical system as effortlessly as possibly by advising patients on available services and appropriate medical follow-up.
  • Makes follow-up phone calls to document health changes and other health related support as needed.
  • Checks and enters diagnostic results in EHR and makes appropriate calls / referrals as directed by the Clinician.
  • Obtains preauthorization for medications and diagnostics as ordered by provider.
  • Performs medication reconciliations.
  • Makes follow-up appointments with practice or specialist, educates patients/parents on when an ER visit is appropriate and the importance of continuity of care.
  • Supports clinical work flow by overseeing Medical Assistants, as assigned.
  • Compliance with HIPAA
  • Compliance with infection control procedures per OSHA
  • Responsible for setting up exam rooms at the start of a shift and restock the room at the end of the shift:
a. Verify all equipment is in good working order.

b. Stock shelves with appropriate medical supplies.

c. Ensure desk files are properly stocked with requisitions and forms.

22. Keep medical supplies arranged and stocked. Provide the Administrative staff with a list of supplies that need to be

replenished (such as gloves, exam table paper, etc). If any equipment is not working, inform the IM Services Manager.

23. Ensure that the bathrooms are properly stocked with supplies.

24. Other duties as specified in the GDCHC / Medical Assistant .

25. Attends meetings as needed.

Requirements:

Successful candidates must possess a high school diploma or GED, hold an active LPN License issued by the State of CT, and two years of appropriate medical office experience (or equivalent).
