The ideal candidate will have a MSW with pediatric Case Management and Palliative Care experience.WHO WE ARE:The Pediatric Advanced Care Team is a newly developed service at the University of Vermont Children\xe2\x80\x99s Hospital. We care for children with serious life-limiting illness and their families, within the hospital and outpatient throughout their illness journey. We are looking to expand the scope of our team and specifically hire someone with the expertise, know-how and enthusiasm to develop and implement a bereavement program for the entire University of Vermont Children\xe2\x80\x99s Hospital.JOB SUMMARY:The incumbent functions as the Pediatric Palliative Care Social Worker for inpatient and outpatient palliative care at the University of Vermont Medical Center and Vermont Children\'s Hospital. Active role in pediatric palliative care consultation as a member of a multi-disciplinary team. Participates in serious illness conversations, including exploring and assessing sources of current and future suffering particularly around the psychologic and social domains, perceptions of prognoses and treatment choices and assistance in plan of care coordination. Responsible for ongoing continuity during hospital course and engaging psychological, social and bereavement support. Attends to family dynamics, assesses and supports coping mechanisms and determinants of health, identifies and facilitates access to resources, and mediates conflicts. Specific focus on bereavement program development and implementation.EDUCATION:Master\'s in social work from an accredited school of social work. LICSW preferred.Palliative Care/Case Management certification by a nationally recognized accrediting body preferred, or willingness to obtain within 3 years; (Examples: CCM, ACM, ANCC or ACHP-SW certifications).EXPERIENCE:1 year of experience in the field of Palliative Care.
