$83,990.40/yr minimum - $125,985.60/hr maximum (range increases based on education, experience and certifications)

Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.

Safety

Record Keeping

\xc3\xaf\xc2\xbf\xc2\xbd\xc3\xaf\xc2\xbf\xc2\xbd Maintains all required records, reports, statistics, logs, files and other documents as required.

Process Improvement

\xc3\xaf\xc2\xbf\xc2\xbd\xc3\xaf\xc2\xbf\xc2\xbd Promotes a culture of process improvement by participating in unit/department based programs that supports the system\xc3\xaf\xc2\xbf\xc2\xbds process improvement goals.
\xc3\xaf\xc2\xbf\xc2\xbd\xc3\xaf\xc2\xbf\xc2\xbd Actively participates on system-wide or hospital-based teams as needed.

Qualifications

Education

\xc3\xaf\xc2\xbf\xc2\xbd\xc3\xaf\xc2\xbf\xc2\xbd Masters Degree in Physical Therapy or Bachelor\xc3\xaf\xc2\xbf\xc2\xbds \xc3\xaf\xc2\xbf\xc2\xbddegree in Physical Therapy with a minimum 5 years of work experience.

Licenses and Certification

\xc3\xaf\xc2\xbf\xc2\xbd\xc3\xaf\xc2\xbf\xc2\xbd Illinois license in Physical Therapy
\xc3\xaf\xc2\xbf\xc2\xbd\xc3\xaf\xc2\xbf\xc2\xbd BLS
