PT Need SNF/Asst. Living Settings - ASAP

Target start: ASAP

13 weeks

40 hrs guaranteed

$70 BR

Will cover two facilities: Heritage Pointe in Warren, IN and Lamplight in Fort Wayne.

Can guarantee 40 hrs if PT covers both locs.

-Drive time and mileage will be reimbursed (mileage reimbursment is .43 cents/mile**)

-Typically 4 days at Heritage Pointe and 1 day at Lamplight Mon-Fri schedule (no evenings, weekends or holidays)

Prefers someone with some experience but will consider new grad

Productivity requirement 80%

Documentation System: RehabOptima

-Traveler will go through extensive training and supervision for evals and documentation. There will be training videos and will work with their clinical team for 1-2 weeks when doing evals.

-Reason: They have had issues in the past where a therapist did not document correctly and the facility did not get reimbursed by gov\'t. They want to provide training to make sure everything is done correctly, no wiggle room for incorrect documentation.

Candidate must be willing to learn, open to feedback and team player!

Heritage Pointe Current Staff: 1 OT, 2 PT, 3 COTAs, 3-4 PTAs, 1 SLP

Lamplight Current Staff: No OT (using one of their clinical consultants to do evals/supervise), 1 PT who works 3 days/week about 3-4 hrs/day

-They also have a daycare and if OT has children, CL will check to see if they can utilize it.
