:PHYSICAL THERAPIST - St. Joseph Home Care Network- Santa RosaSchedule: Full Time, DaysLocation: Responsible for West Santa Rosa area:The Physical Therapist assists the physician in evaluating the patient\'s level of function, develops and updates the plan of care and provides treatment as ordered by the attending physician.Working in Home Health provides the unique opportunity to care for your patients where they are most comfortable. Required qualifications:

• Bachelor\'s degree from an accredited Physical Therapy Program or Master\'s degree from an accredited Physical Therapy Program (Based on Graduation Year) or D.P.T from an accredited Physical Therapy program (Based on Graduation Year).
• California Physical Therapist License upon hire.
• Within 30 days of hire: National Provider BLS - American Heart Association

Preferred qualifications:

• Home Health experience.
• 1 year\' Experience as a Physical Therapist.
