Job Description
We are assisting our client who is currently seeking a Home Health Physical Therapist to join their team. You will treat patients in five counties in Illinois: Kankaee, Will, Grundy, Iroquois, and Ford. The productivity is six acuity adjusted visits per day, with 45 minutes per patient. Compensation and Benefits
- Salary: $35 - $60 per hour based on experience
- Mileage Reimbursement

- Short and long term disability, PSLF Grant eligible
- $1000 CEU
- Provide quality Home Health Physical Therapy services.
- Assist your patients in meeting functional, measurable goals that promote restoration of functional abilities.
- Realize your own potential to achieve your goals and overall excellence.
- Participate in educating the client and the family or other caregivers to promote the client\'s progress toward mutually agreed upon goals.
- Experienced or new PT school grads are encouraged to apply
- Valid Illinois PT License
- Degree from PT Accredited College
