:$5,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment.Physical Therapist- Providence Home Services OregonLocation: Supports McMinnville and surrounding areasSchedule: Full Time/Days with occasional weekendsManages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential.Working in Home Health provides the unique opportunity to care for your patients where they are most comfortable. Required qualifications:

• Bachelor's Degree From an accredited Physical Therapy Program. Or Master's Degree From an accredited Physical Therapy Program. Or D.P.T. From an accredited Physical Therapy Program.
• within 30 days of hire: National Provider BLS - American Heart Association.
• upon hire: Oregon Physical Therapist License.
• 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.
