Job Summary:
Age Requirement: Must be 21 years of age.
The Reentry Case Manager (RCM) will assist all clients of Gastineau Human Services (GHS) to access services related to housing, employment, education, transportation, and substance abuse treatment. RCM\'s also support individuals toward resolving legal issues, improving mental and physical health, resolving family and social issues, and developing healthy life skills.The RCM will work with community partners to assist the clients to access necessary services and benefits. The RCM will assist clients to fulfill their obligations to referral agencies such as Department of Corrections, Office of Children\xe2\x80\x99s Services, court programs while promoting appropriate levels of autonomy and self-determination.As a client advocate, the RCM will engage in respectful, compassionate, fair interactions with clients from diverse backgrounds and help to remove obstacles that impair their ability to achieve their goals. RCMs must have flexible schedules that allow them to provide the highest degree of contact with clients of GHS.
Duties/Responsibilities:

  • Provides intensive case management services during times when most clients are available: referrals, advocacy, coordination of services, and collaboration with other staff and outside service providers to meet client needs.
  • Works with all GHS Clients from entry to discharge.
  • Maintains detailed and accurate case management files and records, intake paperwork, client census, and inputs client information to required data bases.
  • Prepares timely and accurate monthly summaries for each client on caseload for review by the Re-Entry Services Director.

Education and Experience:
  • Must be 21 years of age or older.
  • High School diploma or GED.
  • At least one year of experience in direct human services
Physical Requirements:
  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Must have a current driver\xe2\x80\x99s license and be insurable by current GHS auto insurance carrier.
  • Must pass drug test upon hire.
Department of Corrections Requirements:
  • Must pass a DOC and DOH background check. *Per CRC Standards, these are the minimum qualifications for all employees:
  • HS or GED grad
  • Valid AK Driver\'s License (or obtain one within 30 days of moving to Alaska)
  • Never had a felony
  • Never convicted of Domestic Violence charges
  • Never made, sold, or transported drugs
  • Never had a misdemeanor resulting in physical injury, 2 or more DUI\'s, chemical refusal, or misdemeanor crimes of dishonesty in the past 10 years.
  • Currently not on probation or parole
  • No current pending charges
Schedule and Pay Range:
  • Schedule: 40 hours per week, M-F; with at least one evening shift.
  • Hourly: $28.84 to $40.37 (Annual: $59,982 \xe2\x80\x93 $83,975)
