RN Case Manager
.The primary purpose of this position is to assist the Director of Resident Services with the coordinating and monitoring of services provided to the residents/patients from the time of admission to discharge.
Must possess, as a minimum, a Nursing Degree from an accredited college/university. Must possess a current, unencumbered, active license to practice as an RN in the state of Connecticut.
