Registered Nurse (Maternal Infant Health Program - MIHP) Joy Type: Full-time / Exempt Job Location: Muskegon, MI
Hackley Community Care (HCC) is a Federally Qualified Health Center (FQHC) that offers integrated health services to the residents in Muskegon County and surrounding areas. HCC is looking for a Registered Nurse to join their Maternal Infant Health Program (MIHP). The MIHP is a statewide evidence-based, home visiting program for pregnant people, infants, and their families at no cost to them. Eligible families are partnered with a registered nurse, licensed social worker and registered dietitian. This team dynamic provides care coordination, community resources and intervention services during pregnancy and infancy. The MIHP Registered Nurse will function as a multi-disciplinary team member performing assessments, formulating care plans, providing education and teachings, and managing referrals. The MIHP Registered Nurse will conduct home, office, and virtual visits.

Job duties and responsibilities (not limited to):
Manage internal and external referrals: describe program and schedule assessments.
Perform maternal and infant assessments.
Make any emergency referrals as needed based on initial assessment.
Responsible for maintenance and care for a patient panel of approximately 50 patients (at any given time).
Attempt to ensure maximum reimbursable visits per year.
Documentation requirements within MIHP Electronic Medical Record (Ross).
Communication with HCC staff regarding patients within EMR.
Attend mandatory State meetings.
Job Qualifications:
Possesses a BSN with two years experience in community health, pediatric and/or maternity nursing OR an ASN/AAS diploma with four years of experience in the listed practice settings.
Maintain current, in good standing, nursing licensure from the State of Michigan.
Maintains current CPR certification.
