$16.00 per hour

SUMMARY

Provides ongoing support to residents so that they may acquire the necessary skills to live independently in the community by linking and referring to supportive services which may include but is not limited to housing, treatment for mental illness and/or addiction, employment and/or mainstream benefits. Other needs and resources will be identified to assist residents in maintaining housing stability (food security, health insurance, vocational and educational support, medical treatment, transportation, clothing, social connectedness, etc.). Performs a wide range of tasks including but not limited to training in areas of self-advocacy/assertiveness, daily living skills, community integration, skills development and socialization. Converses with residents to reinforce positive behaviors and to promote social interaction as a role model. Services are individualized to the specific identified needs of each person in a culturally sensitive way. This is achieved using a holistic approach considering several areas of strengths and needs including, but not limited to the individual\'s: history, culture, spiritual preferences, education and employment information, social involvement, mental health and substance abuse status, medical status, safety issues, legal issues and financial issues. Observes and documents resident\'s behaviors to help facilitate assessment of goals. This position may require use of personal vehicle to transport clients when an agency vehicle is not available.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Participates in the oversight of the maintenance of the residence and its conformity to health and safety requirements.

Follows agency policies and procedures in all program areas.

Serves as coordinator of all services the person will receive as described in the housing support plan.

Develops written housing support plan with the resident that is based on strengths and individualized needs.

Reviews, revises and updates the housing support plan every 90 days or as needed.

Links person to all services and supports listed on the housing support plan.

Advocates on behalf of the individual to gain access to needed services and supports in the least restrictive setting.

Maintains adequate and appropriately written progress notes in the case file according to agency guidelines.

Maintains the house in a clean and orderly fashion.

Develops and maintains good community relationships.

Assists program participants with rehabilitation, social, employment and health supports.

Assists in daily living skills development, if needed.

Assists program participants in times of crisis.

Coordinates treatment with other agencies.

Increases use of appropriate community resources.

Encourages participation in resident\'s own housing support plan.

Commences to develop a discharge plan upon enrollment in the program.

Conducts regular record keeping reviews and completes audits according to procedure.

Assists in reviewing, evaluating, and further development of the case management program.

Monitors and maintains high standards of service delivery.

Collects data for statistical purposes.

Maintains client confidentiality at all times.

Reports to supervisor pertinent information concerning illnesses, accidents, untoward events, staff difficulties, repair needs, etc.

Experience and Skills:

EDUCATION and/or EXPERIENCE

Associate\'s Degree in the Human Services field and one year of experience in this field, or a High school diploma or general education degree (GED); and two years related experience and/or training in a human service field. Preferred: Experience with homeless populations.
