Job Title: RN- Assisted Living Program Director Roles & Responsibilities: The position oversees all resident care and services in accordance with policies of the Assisted Living Program ,Physician\xe2\x80\x99s orders, nursing standards of practice and the licensed agency\'s policies, procedures and protocols. This position requires superior communication skills and ability to interact positively with residents and their families.
  • Responsible for the day to day management of the nursing department including scheduling staff, supervision of staff, ensuring appropriate staffing, following up on complaints, etc.
  • Responsible for creating and managing the duty sheets for RSAs for each shift.
  • Responsible for hiring, evaluating ,discipline ,in-service /training program for staffs
  • Coordinates health services between ALP and the certified Home Health Agencies.
  • Responsible for actively participating in the interdisciplinary team meetings.
  • Responsible for ensuring that workflow\xe2\x80\x99s and processes are in place to ensure adequate communication among the staff, i.e. providing reports and responding to emergency calls.
  • Performs screenings of potential residents to determine eligibility for ALP or AH.
  • Responsible for completing or ensuring the completion of the Universal Assessment Tool (currently UAS) on the ALP residents as dictated by the regulations.
  • Completes Patient Review Instrument (PRI) for transitioning residents based on medical necessity to Skilled Nursing Facility (SNF).
  • Maintains effective communication with families, staff, and other responsible parties to ensure compliance with Facility\xe2\x80\x99s policies and procedures.
  • Responsible for the completion of the care plans\xcd\xbe evaluating the resident\xe2\x80\x99s condition on a regular basis and documenting their condition. Informing the physician of a change in status and ensuring a new UAS is completed if needed.
  • Supervises Fall Monitoring program at Facility; reviews and tracks trends and reports outcomes to the Quality Assurance/Performance Improvement Committee.
  • Coordinates readmission process from hospital and/or SNF which may include site visits.
  • Obtain physician orders as needed for any changes to the care plan including medications.
  • Responsible for the supervision of the Medication Assistance Program for all residents who need assistance and for evaluating residents who self-administer medication based on facility policy.
Qualifications:
  • Education: Minimum formal education of Bachelor\'s Degree
  • Certification:Current NYS Registered Nurse license and registration
  • Experience: 3 year job-related experience
