Job Description

In collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the strategies and services required to meet a patient/family\'s end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.

Spectrum Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. Essential Functions:

\xc2\xb7 Communicates effectively with staff, residents and families. Assesses, develops, and maintains an individualized patient plan of care with the patient.

\xc2\xb7 As needed, specified by SH entity: Performs Medication Administration Pass according to regulatory guidelines. Informs MDS coordinator of significant resident/patient changes needed for completion of the MDS.

\xc2\xb7 Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.

\xc2\xb7 Evaluates patient\xe2\x80\x99s and significant other\xe2\x80\x99s educational needs and develops and implements appropriate teaching strategies or makes appropriate referrals.

\xc2\xb7 Observes and reports the patient\xe2\x80\x99s condition, needs and outcomes of intervention to the physician and other team members. Identify problems and emergency situations, and initiate immediate \xe2\x80\x9clife saving\xe2\x80\x9d measures in the absence of a physician.

\xc2\xb7 As needed, specified by SH entity: Teaches and supervises the care provided by CNA, LPN or other nursing service personnel according to regulatory and agency guidelines.

\xc2\xb7 Implements the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures, regularly evaluating the patient\xe2\x80\x99s progress toward established goals recommending revisions to the plan as necessary, and planning for termination of service or discharge. Assumes a leadership role and delegates appropriately to support staff to facilitate safe, quality patient care in a cost-effective manner.

\xc2\xb7 Participates in ongoing process improvement activities through the application of clinical paths, QA processes and other related activities. Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency.

Qualifications:

\xc2\xb7 Associate\'s Degree Nursing \xe2\x80\x93 Required

\xc2\xb7 Bachelor\'s Degree Nursing \xe2\x80\x93 Preferred

\xc2\xb7 1 year of relevant experience of acute care, emergency, or rehab facility experience - Preferred

\xc2\xb7 Registered Nurse (RN) in State of Michigan required upon hire

\xc2\xb7 Driver\'s License in State of Michigan required upon hire
