As a Home Health Registered Nurse, here\xe2\x80\x99s a big-picture view of what you\xe2\x80\x99ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases\xe2\x80\x94often after discharge from a hospital, rehab or nursing facility
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
• Examine patients, do assessments, document progress and report changes to their physician
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
• Coach family members on the patient\xe2\x80\x99s plan of care, medication and home safety
• Ensure goals are met and coordinate discharge from services

A few must-haves for Home Health Registered Nurses:

• Active Registered Nurse (RN) License in Colorado
• Graduate of an accredited school or program of nursing
• At least 1200 hours of experience as an RN
• Health care CPR/BLS Certified (demonstration course required)
• Home health care experience preferred
• Pass a background check and drug test (we are required to test for THC)
• No Covid Vaccination Required
