Job Highlights:
The role includes working as a medical imaging RN and Breast Navigator (Coordinator) at both Waupun Memorial Hospital and Ripon Community Hospital.
Medical Imaging RN
- Assist the radiologist and staff on procedures performed in the medical imaging department. For example, Paracentesis, Thoracentesis, Biopsies, etc..
- Assists with Diagnostic Mammograms - Breast Coordinator is contacted, speaks with patient, sets up biopsy, and contact provider for order.
- Stereotactic or Ultrasound Guided Biopsies - After the procedure Breast Coordinator does discharge instruction. F/U phone call next day by Breast Navigator to check how patient is doing. Bx results are usually available within 48 hours.
- Surgery - Scheduled by surgeon\xe2\x80\x99s office (Sometimes this is coordinated with plastic surgery) - Breast Navigator sees the patient on the day of surgery in the hospital (sees pre/post op for day surgery patients). Facilitates order of a post op camisole (if patient desires and has mastectomy without reconstruction). Coordinates with the Health Shoppe to have it delivered before discharge. This requires an MD order and fax insurance information. (Note that most are lumpectomy only pts and this is not needed). Breast Navigator see the patient day and discharge and assists with fitting the camisole. Post op call to all surgical patients after discharge.
- Chemotherapy and/or Radiation Therapy - Breast Navigator accompanies patient to 1st chemotherapy treatment if able and calls after every treatment. Patient on radiation are called about every two weeks. Patient is called 1 month after a hormone blocker is started.
- Additional duties - Additional points: All encounter and conversations are documented in EPIC, all Pathology entered into EPIC by Breast Navigator, referral to the American Cancer Society are made upon patient request for a support person\xe2\x80\x94the forms are completed and faxed to ACS and the will match the patient with a support person and Breast Biopsy log in Medical Imaging (mammo office). With all results and addendums. The Imaging Department provides a quality report regarding the # of patients with recommendation from the radiologist and the number of benign or malignant findings (MQSA requirement).

Job Summary: Provides services as a patient navigator throughout the continuum of care.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
- Collaborates with physicians and advanced practice providers to ensure quality outcomes. Coordinates efforts for treatment and promotes quality improvement and educational efforts.
- Compiles data, tracks outcomes, and makes recommendations for process improvement.
- Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
- Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.
- Assists medical provider with processing of diagnostic results bringing significant values to the provider\xe2\x80\x99s attention. Exhibits knowledge of tests and procedures and their significance as related to patient care.
- Recognizes the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients. Uses knowledge of medications in instructing patients about medication therapy.
- Establishes working relationships teaching protocols for patient education, family counseling and general public information.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
- Graduate of accredited school of nursing or education equivalency for licensing
- Three years\' registered nurse experience
- Constant use of speech to share information through oral communication.
- Constant standing and walking.
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, reaching and keyboard use/data entry.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of smell to detect/recognize odors.
- Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Occasional lifting/moving of patients.
- Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
- Occasional driving.
- Rare crawling and running.
