The Service Coordinator, reporting to the Director of Operations, ensures compliance with Home and Community-Based Services (HCBS) for ID, BI and HAB waiver regulations. This role facilitates new member admissions, gathers referral information, initiates intake processes, and coordinates seamless onboarding. The Service Coordinator also connects members to appropriate community resources, ensures services align with Individual Service Plans (ISPs), and provides exceptional customer service to members, their care teams, and the public. This position thrives in a fast-paced, dynamic environment.
Key Responsibilities:
Ensure the health and safety of members by evaluating programs for alignment with individual needs.
Conduct intakes, screenings, and assessments for Fast-Trans.
Coordinate the development and implementation of ISPs, assessments, and key service documents, ensuring timely updates based on service needs.
Perform monthly face-to-face visits with members to assess services and foster meaningful relationships.
Responsible for the health and safety of the members as well as evaluating the program for its ability to meet individual needs of each consumer.
Serve as the primary contact for members, caregivers, and external stakeholders.
Partner with House Leads to ensure effective implementation of services as outlined in ISPs.
Collaborate with the Quality Assurance team to implement recommendations promptly.
Assist in internal and external referrals to connect clients with appropriate programs and resources.
Facilitate quarterly and annual meetings.
Maintain accurate and compliant client files, medical records, and service documentation.
Review, remediate, and follow up on incident reports.
Create, update, and manage addendums and service plan revisions as needed.
Coordinate member onboarding, including transportation, medication pick-up, and house transitions.
Provide on-site training and participate in staff meetings as required.
Responsible for facilitating clients quarterly progress reports.
Collaborate with Quality Assurance in completing yearly surveys for clients and stakeholders.
Other duties as necessary or as assigned.

Pay: $16.00 - $19.00 per hour
