Ray Graham Association for People with Disabilities is seeking a

Service Intake Coordinator (SIC).

The Service Intake Coordinator is responsible for the centralized creation, management, and oversight of the agency referral and intake process. This role is essential in maximizing opportunities for people seeking services and their families to access the organization's service offerings. The SIC will play a pivotal role in ensuring flawless coordination and direction of calls/service inquiries, contributing to the overall efficiency and effectiveness of the organization's delivery service. This position supports the mission and values of the Ray Graham Association and demonstrates Person-Centered Excellence in all interactions with service seekers.


Requirements:
Develop and oversee a centralized referral/service inquiry process to ensure efficient and effective service access. Direct the intake processes and procedures seamlessly across all service areas of Ray Graham Association. Develop relationships with referral sources and identify potential candidates for identified vacancies. Must be knowledgeable of the Illinois Department of Human Services (IDHS) Prioritization for Urgency of Need for Services (PUNS) database. Must be knowledgeable of IDHS Division of Developmental Disabilities (DDD) programs, bill codes, client types and waiver indicators. Bachelor's degree in relevant field required/ master's degree preferred Valid Driver's License required Bilingual English/Spanish preferred

Schedule:


Monday - Friday 9-5 (Must be available for some evenings and weekends)
