Overview



The Youth Homeless Case Manager works with transition-age youth (ages 18- 24) with substance abuse and/or emotional and behavioral disorders to facilitate their movement towards greater self-sufficiency and successful achievement of their goals, one of which will be to obtain and maintain supportive housing. The Case Manager will engage each youth in an individualized process, engaging them in their own futures planning process and provide them with developmentally appropriate services and supports while supporting them to obtain supportive housing. Family and other supports will be encouraged to actively engage in the process. This position will be working with transition age youth referred through the Anchorage Coalition to End Homelessness\xe2\x80\x99s Continuum of Care program (CoC) and other community stakeholders.

Essential Functions, Duties & Responsibilities

• 
  
  Engages youth assigned to caseload through relationship development, person-centered planning and focusing on their futures using a strength-based approach.

• 
  Implements the TIP model to fidelity to ensure positive outcomes.
• 
  Works primarily in the community connecting clients to resources and practicing life skill to assist them in becoming self-sufficient.

• 
  Tailor services and supports in accordance to the TIP model and builds on strengths to enable youth to pursue their goals in the transition domains.
• 
  Facilitates futures planning, goal setting and prevention planning for high-risk behaviors and situation to improve functioning and maintain housing placement.
• 
  Acknowledges and develops personal choice and social responsibility.
• 
  Facilitates the development and use of problem-solving and decision-making skills.
• 
  Involves the youth\xe2\x80\x99s parents, family members and other informal and formal key supports to create a safety net.

• 
  Coordinates services with community partners and referral sources to include covenant house, probation officers, Neighbor Works, Anchorage Coalition to End Homelessness\xe2\x80\x99s Continuum of Care program (CoC),Office of Children Services representatives, Covenant House Alaska and other involved providers, parents and family members.
• 
  Participates in weekly case review meetings.
• 
  Participates in training on an initial and on-going basis.
• 
  Completes clinical documentation as mandated by agency policies, Medicaid regulations, CARF standards and the Division of Behavioral Health grant conditions.
• 
  Schedules treatment plan review meetings every 120 days with youth assigned to caseload.

• 
  Maintains client confidentiality and safeguards client information per 42 C.F.R. Part 2 and HIPAA privacy and security regulations.
• 
  Review treatment curriculum and/or interventions and updating quality and materials as needed
• 
  Consult on cases and treatment plans and provide client advocacy.
• 
  Must carry at least 20 hours(Full time staff) or 10-15 hours(Part time) of direct clinical care weekly, and be responsible for the following:

• 
  Demonstrate ability to provide comprehensive and ongoing Case Management including coordination of services, linking to needed supports for housing, providing daily living skills training and support resulting in informing treatment team with appropriate treatment and discharge recommendations.

• 
  Follow individualized treatment plan created by clinician with specific and measurable goals. Must continually monitor progress and re-evaluate appropriateness of goals, resetting as appropriate in consultation with Directing Clinician.
• 
  Demonstrate knowledge of and practice of clinical interviewing/counseling skills.
• 
  Ensure that patients are seen for services as clinically indicated (on assessment/treatment plan).
• 
  Encourage reluctant and/or resistant patients and/or family members to stay in program and build motivation. Work with supervisees to do the same with their caseloads.
• 
  Be creative in therapeutic interventions to include homework assignments, community resources, etc.

• 
  Conduct Utilization Reviews and ensure supervised staff are conducting in timely fashion to ensure payment.
• 
  Have ongoing attention to discharge plan.
• 
  Effectively communicate to other members of the treatment team about patient care and treatment.

• 
  Services to be provided will include but not limited to: Case Management, Intensive Case Management and Crisis Recovery Supportive Services.
• 
  Functions as a full member of the multidisciplinary treatment team and works collaboratively with all team members to ensure transition age youth are receiving effective services in accordance with their assessment/treatment plan.

• 
  Reviews Critical Incidents, Treatment Plans, Staffing Summaries, and Assessments of assigned staff to ensure quality care and compliance to policy and regulation.
• 
  Participates in the development of individualized treatment plans and treatment plan reviews using the outcome measures screening data.
• 
  Responsible for communicating important clinical data to appropriate team members.
• 
  Maintains client confidentiality and safeguards client information per 42 C.F.R. part 2 and HIPAA privacy and security regulations.
• 
  Expected to complete all assigned duties in accordance with timelines established by immediate supervisor.

• 
  Must carry at least 20 hours of direct clinical care weekly, and be responsible for the following:

• 
  Demonstrate ability to provide comprehensive and ongoing Intensive Case Management service including coordination of services, linking to needed supports for housing, providing daily living skills training and support resulting in informing treatment team with appropriate treatment and discharge recommendations.
• 
  Follow individualized treatment plan created by clinician with specific and measurable goals. Must continually monitor progress and re-evaluate appropriateness of goals, resetting as appropriate in consultation with Directing Clinician.

• 
  Provides home, school and community-based services.
• 
  All other duties as assigned.

Knowledge, Skills and Abilities

• 
  
  Bachelor\xe2\x80\x99s Degree in the Human Services field ( preferred) and a minimum of 2 years working with persons with severe emotional disturbances, substance use and behavioral disorders

• 
  Proven ability to work with diverse community groups; with a focus on Alaska Native people, knowledge of their values and belief systems and ability to provide culturally competent services
• 
  Ability to make sound decisions based on information available.
• 
  Excellent writing and communication skills
• 
  Ability to work flexible hours on occasions, including days, evenings, and some weekend hours
• 
  Ability to travel if required. Must have own transportation and proof of insurance 100/300.

• 
  Ability to provide current credentials and if not current, willing to work towards certification requirements and obtain QAP within 3 years of hire.
