Job Description


Responsibilities

Under general supervision, provides assistance to mental health therapists, case managers, and other hospital personnel in provision of direct services to patients. Plays a key role in assessing patient\'s post hospital care / continuity of care needs and connecting patients/families with community resources. In collaboration with clinical teams, assumes responsibility for the provision of concrete resources such as housing, transportation, food assistance, etc. Acts as a content expert in community resources and services to support patients, families, and the larger organization.

Job Posting Addendum

This is a 0.9 FTE (72 hours per bi-weekly pay period) Night Shift (7:00pm-7:00am) position on variable days including every other weekend.

Requirements

Required Education/Experience:

  • Bachelors degree in a social science or a related field.
  • Experience working with families with medically or behaviorally complex needs.
Required Credentials:
  • Agency Affiliated Counselor Registration in Washington State within 6 months of employment.
  • Registration as a Nursing Assistant in Washington State within 4 months of employment.
  • Basic Life Support (BLS).
  • Safety Care certification (internal training) required within 3 months of hire and annually thereafter.
Preferred:
  • Experience working with children or adolescents in group settings.
  • Working knowledge of entitlement programs, including Medicaid, Medicare Social Security Disability, WA State DSHS benefits/programs, etc.
Min to Max Hourly Salary

$30.37 - $45.55 /hr

Min to Max Annual Salary

$63,169.60 - $94,744.00 /yr

Salary Information

This compensation range was calculated based on full-time employment (2080 hours worked per calendar year). Offers are determined by multiple factors including equity, skills, experience, and expertise, and may vary within the range provided.
