Job Description

Pay Rate:

$20 per hour



The Social Worker / Case Manager is involved in counselling and assisting clients to meet personal, social, health, and economic needs.



Job Responsibilities:



1. Collaborates with individuals with disabilities and aging population

2. Assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.

3. Make recommendations as part of a Client Service Plan.

4. Interpreting laws, rules, regulations, standards, policies & procedures.

5. Narrative report writing.

6. Assist with gathering documents & monitoring the process until a determination is made.

7. Makes referrals and assists clients in applying for a variety of services and/or programs.

8. Maintains a caseload, determines initial and ongoing eligibility for services under the Community Services Program.

9. Complete activities by the due date.

10. Create activities in companies Aging & Disability database as outlined in Job Aid.

11. Respond to routine calls/emails within 2 business days of contact.

12. Utilize companies Aging & Disability database, Outlook Calendar, Tracking Assessing Planning (TAP), ASSIST Worker Web (AWW), Vital Statistics, and Delaware Medicaid Enterprise System, (DMES), for casework activity.

13. Check New Referral saved search daily to retrieve referral.

14. Contact participant to complete a application over the phone. Monitor application process from start to decision.



Job Requirements:



1. Must have 6 months or more of Case Management experience.

2. Must have narrative writing skills.

3. Must be proficient in Microsoft Suites, Word and Excel.
