:The Social Workers (LCSW or LMFT) evaluates and assesses patient needs for community resources, end of life planning and coordination of clinical programs and acts as a liaison between the patient, hospital, Agency, Physician, Insurance Plans and community with in the Providence Home Care Service Lines. Embraces and utilizes evidence based assessment and patient education tools. • Master\'s degree in Social Work
• California Clinical Social Worker License upon hire or California Marriage and Family Therapist License upon hire
• 1 year of Social Work experience.

Preferred qualifications:

• Experience in home health care setting.
