:Provides education and support services to patients, families and staff. Provides psychosocial assessment, counseling, crisis intervention, referrals and information to patients and families. As a member of the Case Management Department, works collaboratively with the interdisciplinary team to effect positive patient and family outcomes. Required Qualifications:

• Master's Degree Social Work from an accredited graduate school of social work. And
• upon hire: Texas Masters Social Worker License - Texas State Board of Social Worker Examiners or upon hire

Preferred Qualifications:

• 1 year Minimum of one year experience delivering social work services in a healthcare setting.
• Post degree experience in a hospital or other health care setting.
