:This position will be traveling to see patients primarily in Tacoma/Pierce county.The MSW Social Worker is responsible for providing services to hospice patients, families, and caregivers, addressing a wide range of social work care coordination and psychosocial issues related to end-of-life needs including: adjustment to illness/prognosis, care system issues, community resource issues, insurance/financial issues, legal/ethical issues, and end-of-life planning. This position develops and implements a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG) and is responsible for an assigned caseload shared with an RN Case Manager and a Hospice HA with the goal of promoting patient and family choice, ensuring continuity of care, and enhancing the quality of life of dying patients.Providence caregivers are not simply valued - they're invaluable.
