:This position will be traveling to see patients primarily in Tacoma/Pierce county.The MSW Social Worker is responsible for providing services to hospice patients, families, and caregivers, addressing a wide range of social work care coordination and psychosocial issues related to end-of-life needs including: adjustment to illness/prognosis, care system issues, community resource issues, insurance/financial issues, legal/ethical issues, and end-of-life planning. This position develops and implements a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG) and is responsible for an assigned caseload shared with an RN Case Manager and a Hospice HA with the goal of promoting patient and family choice, ensuring continuity of care, and enhancing the quality of life of dying patients.Providence caregivers are not simply valued - they're invaluable. • Masters Degree from an accredited School of Social Work.
• Washington Advanced Social Worker License upon hire. Or
• Washington Advanced Social Worker Associate License upon hire. Or
• Washington Clinical Independent Social Worker License upon hire. Or
• Washington Clinical Independent Social Worker Associate License upon hire.

Preferred Qualifications:

• 1 year of Post-Masters work experience
• 1 year of prior internship experience in a Providence Health System setting
• 1 year of prior experience in a healthcare setting
