The Medical Social Worker conducts psychosocial evaluations and formulates treatment plans and goals to assist with resolving problems and supports participants in meeting their biopsychosocial needs. Makes referrals to appropriate community resources. Confers with other disciplines. Participates in internal meetings. Completes documentation in a timely manner. Assists in community outreach efforts in conjunction with the Program Director and Counseling/Social Work Manager.

Required qualifications:

• Bachelor\'s degree in Social Work from a school of social work accredited by the Council on Social Work Education (CSWE), or in Psychology, Sociology, or other field related to social work.
• Master\'s degree in Social Work from a school of social work accredited by the Council on Social Work Education (CSWE).
• 1 year\' Clinical experience in a health-care setting.

Preferred qualifications:

• 1 year\' Social work experience in a health care setting.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

About the Team: Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
