Harlem Hospital Center has been a bedrock of the Harlem community since 1887. Duties & Responsibilities
Under the direction of the Supervisor, the Social Worker is responsible for providing Crisis Intervention services, including psychiatric and psychosocial assessments, to people within the community who are experiencing a psychiatric crisis. SUMMARY OF DUTIES & RESPONSIBILITIES:
Conduct comprehensive psychiatric and psychosocial evaluations, in the community, by visiting a person\xe2\x80\x99s home, supportive housing facility, or shelter. Visits are conducted in a timely manner from receiving the referral and occur during the day any time based on the work schedule including evenings, weekends, and holidays.
Provide Crisis Intervention services which includes de-escalation, linkage to resources, or initiating transport to the hospital, if needed.
Obtain collateral information from patient\xe2\x80\x99s family, housing facility staff, or community members for the purpose of assessing a patient\xe2\x80\x99s ability to function in the community.
Provide follow up to confirm linkages are made between the patient and service provider/agencies to ensure continuity of care.
Document all relevant information, in a timely manner, into the patient\xe2\x80\x99s medical record.
Carry a case load and adhere to the Department documentation policies and procedures.
Complete, maintain, and submit required reports in a timely manner.
Report suspected cases of child abuse or neglect to the New York State Central Registry.
Uphold NYC Health+ Hospitals, customer services standards when interacting with patients and staff.
Adhere to patient confidentiality expectations as per HIPAA guidelines.
Maintain infection control protocol.
Comply with appropriate patient safety standards when providing patient care, as per Corporate and Network Policies.
Knowledgeable of departmental, facility, city, state, federal and TJC standards and regulations.
Performs other functions as assigned by Behavioral Health/Social Work administration.
Minimum Qualifications
For Assignment to Level II 1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED.
