Sibley Memorial Hospital is a non-profit, full-service 318-bed acute care community hospital serving the Washington, DC area. Sibley offers medical, surgical, intensive care, obstetric, oncology, orthopedic and skilled nursing inpatient services, as well as a 24-hour Emergency Department. Sibley\xe2\x80\x99s campus is also home to Grand Oaks, an assisted living residence; a medical building with physician offices and ambulatory surgery and imaging centers; and the new patient tower with 200 spacious, private rooms.You were meant for Hopkins.Sign-on Bonus $5,000.00SUMMARY: The Palliative Care Social Worker is responsible for conducting psycho/social assessment/evaluations. The Palliative Care Social Worker will collaborate with professional palliative care team to review patient care; provide casework and group work for patients and their families; identify community resources, and assist patients and their families in connecting to these resources. This individual serves as a multidisciplinary patient care team member with liaison responsibilities with the hospital and staff members of community organizations and other healthcare organizations.EDUCATION AND EXPERIENCE REQUIRED: * Licensed to practice as a Social Worker in the District of Columbia. Master\xe2\x80\x99s Degree in Social Work from an accredited school.

  • Minimum of 2-5 years of social work experience within palliative care
