***Substance Abuse Disorder Counselor 24hrs/Evenings

POSITION SUMMARY:

The Substance Use Disorder (SUD) Counselor assesses patients with SUD for continuing recovery needs, participates in the formulation of an interdisciplinary plan of care, makes recovery support recommendations to patients based on their history of use and prior treatment, provides patient psychoeducation in individual and group sessions and makes referrals for continued recovery treatment.

KEY RESPONSIBILITIES:

The Substance Abuse Counselor will:

  • Conduct initial counseling psych/social evaluations including strengths, needs, abilities, preference for aftercare, addiction and development and psychiatric history and assessment
  • Assists the patient to develop long and short-term goals as part of their interdisciplinary, individualized treatment plan of care.
  • Identifies co-existing conditions and implications for treatment.
  • Conduct daily individual counseling session with assigned case load (up to 8 patients) to evaluate progress in treatment, provide education and plan for discharge.
  • Conduct family meeting sessions.
  • Identify resources and refer patient to community service provider, PCP, on-going Substance Use Disorder treatment, and other sources of support and education.
  • Informs patient of treatment process, and aftercare options.
  • Promptly contacts other professionals involved in treatment for exchange of information.
  • Facilitate and lead group counseling sessions (20-25 patients) utilizing various counseling approaches and formats (didactic, interactive, engagement worksheets) on scheduled addiction topics such as relapse prevention, self-help, triggers, feelings, denial, and stress management
  • Identify and complete all necessary Releases of Information to complete aftercare arrangements in accordance with Steward policy and Federal/State law.
  • Develop a support plan for each patient including education and identification of local self-help meetings.
  • Advocates for patient rights.
  • Adheres to policies regarding confidentiality.
  • Participates as a member of an interdisciplinary team to manage patient crisis.
  • Participates in the rotation of Charge Counselor assignment.
  • Maintains Professional Boundaries with patients.
  • Performs other duties as assigned.

  • Responsible for ongoing development of his/her work skills using available resources (i.e.: Steward University, inservices, formal educational programs, other work groups and on-the-job training).
  • Assists in the orientation of others and actively participates in mentoring.
  • Recognizes and seeks assistance/consultation when appropriate.

REQUIRED KNOWLEDGE & SKILLS:
  • Good communication and interpersonal skills
  • Ability to create a friendly, caring environment for patients, families, visitors, and staff.
  • Displays willingness and flexibility in learning new functions
  • 42 CFR \xe2\x80\x93 Part 2 law governing patient confidentiality in Substance Use Disorder Treatment Programs.
EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:

Education:
  • Bachelor\'s degree in human services, counseling, psychology, social work or related field required.
Experience:
  • One (1) experience in Substance Use Disorder counseling role (including internship experience);
  • Inpatient experience preferred.
Certification/Licensure:
  • CADAC or LADAC strongly preferred
Software/Hardware:
  • Basic computer skills required.
  • Experience with EMR, particularly Meditech, preferred.
