Woodland Manor Nursing and Rehab is seeking a

Temporary Social Services Director

for our nursing home in Elkhart, IN. This opportunity is ideal for candidates with skilled nursing/long-term care (LTC) experience who are looking for a short-term, impactful role in a supportive environment.

Key Responsibilities:

Develop social assessments and care plans to address medically related social and emotional needs, setting realistic goals and actions. Actively participate in pre-admission and admission processes. Provide compassionate support to residents and family members during times of adjustment, crisis, or special needs. Document residents' behavioral and psycho-social status effectively and accurately. Facilitate individual and group support for residents and their families. Create and maintain a volunteer program tailored to meet specific social service needs. Orient residents and families to the facility, ensuring a smooth transition and positive adjustment.

Qualifications:

Bachelor's Degree in Social Work or a related field from an accredited institution. Strong knowledge of social services practices, procedures, laws, and regulations governing long-term care. Compassionate listener and communicator with the ability to work independently.
