WHAT DOES YOUR DAY ENTAIL? This Transition Specialist with our human services nonprofit is dedicated to supporting individuals in their journey toward successful re-entry into the community. Each day you conduct thorough assessments, using motivational interviewing techniques to develop personalized transition plans that meet the unique needs of each participant. These person-centered plans serve as roadmaps for their successful reintegration. Crisis intervention and safety planning are crucial aspects of your role. You remain vigilant, providing immediate support when participants face challenging situations. Drawing on your evidence-based clinical knowledge, you offer clinical support as needed, ensuring that individuals receive the care they require. Another key responsibility is facilitating care from Rikers Island to the community. You actively collaborate with internal and external stakeholders, such as the Department of Correction (DOC), Correctional Health Services (CHS), jail-based providers, defense teams, and community partners. By establishing strong connections and coordinating services, you ensure that participants have access to the appropriate resources and support networks. Regular presentations of services within the jail are also part of your routine. You engage with detainees, informing them about the program and the available resources to aid their transition. This proactive approach helps individuals understand the opportunities that await them upon release. Maintaining consistent communication is vital. You reach out to participants through face-to-face meetings, phone conversations, emails, and text messages, ensuring that they are actively engaged and meeting program standards. You document their progress, complete transition plans, and report outcomes to track their achievements and program effectiveness. In collaboration with the Reentry Mentors and Reentry Coordinator, you ensure a seamless transition and continuity of care for participants. By working closely with the team, you collectively support their successful reintegration into society. Your day also includes attending meetings, participating in professional development training, and engaging in regular supervision sessions. This continuous growth and learning enable you to enhance your skills and stay up-to-date with best practices in the field. Throughout your day as a Transition Specialist, you enter case notes for each job seeker in FedcapCARES, ensuring accurate documentation and maintaining organized participant files. This comprehensive approach allows for effective tracking of progress toward goals and efficient reporting. You love working as a transition coordinator where you play a vital role in empowering individuals and making a positive impact on their lives.
