GENERAL SUMMARY/ OVERVIEW STATEMENT:
The Transplant Coordinators report directly to the physician of the transplant programs with administrative oversight by the Transplant Center Director or their designee. The coordinators are responsible for ensuring patients\' continuity of care after their discharge phase of transplantation. They serve as the central point of reference for all information about patients participating in the program. The scope of the position includes patient care, education, quality assurance, research, administrative and financial activities, and coordination of transplant care with medical and surgical staff as appropriate.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Care
Post-Transplant Discharge Phase
\xc2\xb7 Provides care, within his or her scope of practice and licensure in the outpatient setting to achieve optimal physical, social, and emotional rehabilitation.
\xc2\xb7 Identifies and responds to recipient/family/legal guardian\xe2\x80\x99s educational, psychosocial, and economic needs in the post-transplant phase.
\xc2\xb7 Reinforces education and follow-up requirements necessary for successful outcome.
\xc2\xb7 Observes, monitors, and assesses recipient\xe2\x80\x99s compliance with medications and medical regimen; collaborates with the transplant team to attain and maintain compliance.
\xc2\xb7 Collects and reviews data pertinent to recipient\xe2\x80\x99s health and organ function and ensures that the information is documented in the patient\xe2\x80\x99s chart/OnCall.
\xc2\xb7 Communicates lab data and diagnostic results to transplant physician/health care provider.
\xc2\xb7 Implements changes in drug/treatment therapy as ordered by physicians and other credentialed health care providers.
\xc2\xb7 Understands post-transplant protocols and acts as a resource for internal and external health care providers.
\xc2\xb7 Arranges for consultations, diagnostic procedures and hospitalization when indicated.
\xc2\xb7 Maintains communication with referring physician, and other health care providers.
\xc2\xb7 Provides and maintains comprehensive documentation in patient medical record.
\xc2\xb7 Identifies and responds to the educational, psychosocial, and economic needs of transplant recipients/live organ donors and families during the post-transplant/follow-up phase.
Research
\xc2\xb7 Examines current transplant-related research.
\xc2\xb7 Participates in and supports research activities within the Transplant Program and MGH Transplant Center and publishes results as appropriate.
Education
\xc2\xb7 Initiate, participate, and contribute to education programs for staff at the unit/service/Center levels.
\xc2\xb7 Maintain clinical skills and certification as appropriate to role.
Administrative and Financial
\xc2\xb7 Participate in developing, implementing, and evaluating on an ongoing basis guidelines, protocols, and standards.
\xc2\xb7 Maintains system of record-keeping on all potential transplant candidates and liver organ donors and transplant recipients and organ donors.
\xc2\xb7 Is knowledgeable regarding requirements for documentation, data submission, quality assurance and other reporting of UNOS, NEOB, CMS and other regulatory bodies and ensures compliance with Policy and Procedure for Data Submission to the OPTN.
\xc2\xb7 Maintains monthly and annual statistics including summaries of clinical data and postoperative follow-up care.
\xc2\xb7 Collaborates among MGH organ-specific transplant programs to ensure efficient and effective use of resources and expertise.
\xc2\xb7 Initiate and participate in required quality assurance activities.
\xc2\xb7 Participate on committees and task forces that relate to organ transplantation as requested by Physician Director(s) and/or Transplant Center Director (or designee).
\xc2\xb7 Maintain data in clinical records and databases as appropriate to evaluation/clinical care and consistent with Transplant Center/organ-specific policies and procedures.

\xc2\xb7 Participates in professional transplant/procurement organizations.
\xc2\xb7 Shares knowledge through mentoring/precepting other Transplant Coordinators and transplant staff as needed.

Working Conditions:
Work will be performed in an inpatient and outpatient clinic setting. The work may include exposure to potentially infective material. Hazards present from exposure to combustible anesthetic or disinfecting agents are minimized when safety precautions are strictly adhered.
SUPERVISORY RESPONSIBILITY:
No direct supervisory responsibility required; however, works cooperatively and provides some day-to-day direction to Patient Service Coordinators.
FISCAL RESPONSIBILITY:
No direct fiscal responsibilities; however, is responsible for prudent fiscal management of program resources, and when determining evaluation tests, etc.
Qualifications
Qualifications:
Current Massachusetts licensure as a registered nurse
Bachelor\xe2\x80\x99s degree in nursing or equivalent experience required.
Master\xe2\x80\x99s degree in a clinical nursing specialty preferred.
Minimum of 3 years recent Intensive Care Unit and/or transplant experience
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

\xc2\xb7 Demonstrates ability to clinically assess and monitor live organ donors using clinical assessment skills (if applicable).
\xc2\xb7 Demonstrates ability to monitor for, and identify complications related to organ transplantation rejection.
\xc2\xb7 Demonstrates ability to monitor for, and identifies complications related to organ donation (if applicable).
\xc2\xb7 Demonstrates knowledge of and identifies complications related to organ transplantation: infection, immunosuppressive therapy, post-transplant diabetes.
\xc2\xb7 Demonstrates ability to provide patient education related to signs and symptoms of organ rejection and complications following living organ donation (if applicable).
\xc2\xb7 Demonstrates ability to monitor immunosuppressive therapy under the supervision of a qualified physician.
\xc2\xb7 Demonstrates ability to provide patient education regarding immunosuppressive therapy.
\xc2\xb7 Demonstrates ability to identify the educational, psychological, and economic needs of recipient/significant others and incorporate into the multidisciplinary care plan.
