StartDate: 3/3/2025 Available Shifts: 8 D Pay Rate: $2599.77 - $2848.68Physical Therapist - Home Health needed asap for 13 weeksTerritory: Sacramento, Ca (Will be required to expand as needed)Schedule: 40 hours per week, Monday-Friday and one weekend a month rotationTraining: 3 full days, one in house and two in the fieldDocumentation System: KinnserEquipment Issued: iPad, Verizon One TalkMileage Reimbursement: $0.65.5/mile (from first to last patient)Parking and Toll Reimbursement: PaidCompany Car: Option with a decreased bill rateRequirement: CA PT License and one year of Home Health experiencexe2x80xa2 Required to prioritize start of care visitsxe2x80xa2 Required to supervise Physical Therapy Assistantsxe2x80xa2 Next day mandatory reporting of start of care visitsxe2x80xa2 Clinical notes and other documentation due within 48 hours of visitxe2x80xa2 Maintain timely communication with supervisor, intake, and officexe2x80xa2 Be generally responsive during normal business hoursRequired Qualifications
