Job Responsibilities

With our Agency\'s mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina.

This position is located at the Department of Mental Health, G. Weber Bryan Psychiatric Hospital, 220 Faison Drive, Columbia, SC 29203.

As the Treatment Team Coordinator/Social Worker, you will coordinate Treatment Planning Services for the patients of GWBPH. You will provide consultation to the Treatment Team coordinator to ensure that all plans are created in conjunction with the patient and meets Joint Commission, CMS and best practice standards. These duties include creating and maintaining ongoing treatment plan update schedules, creating initial treatment plans, facilitating annual updates, ensuring goals are clear, concise and measurable and working with all disciplines to generate compliant goals, interventions and objectives.

Responsibilities for the Treatment Coordinator/Social Worker include but are not limited to:

  • In conjunction with the Director of Social Work, LPP\'s and Medical Director you will develop a treatment team schedule for each lodge to be managed by the Treatment Team Coordinator(s).
  • Manages the creation and maintenance of treatment plans for each patient on the BPH Adult campus.
  • Works with the Director of Clinical Treatment Services and Social Work Director to ensure that all Joint Commission, CMS and best practice standards are met in the develop of written plans of care.
  • Serves as Treatment Team Coordinator for each BPH Adult/Civil lodge. In conjunction with Treatment Team members, ensures that each patient has an individualized treatment plan that encompasses all pertinent and measurable goals, interventions and objectives.

Minimum and Additional Requirements

Minimum Requirements: A bachelor\'s degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board.

Agency (Facility) Minimum Requirements: A master\'s degree from a Social Work program accredited by the Council on Social Work Education, LMSW-SC Board of Social Work Examiner\'s and three (3) years of practice experience at this level. Must be licensed by the SC Board of Social Work Examiner.

Note: If any Agency Requirements are listed above, applicants must also meet those requirements to be considered for the position.

Additional Comments

The applicant must have thorough knowledge of Federal and State standards pertaining to social services provision. The applicant will be required to be licensed by the SC Board of Social Work Examiners at the LMSW or LISW-CP level. Ability to move freely around campuses to complete essential job duties. Computer knowledge and skills sufficient to complete essential job duties in an efficient manner. The applicant should be capable of delivering social work services to a diverse and highly vulnerable population which presents with multiple psycho-social problems, with little supervision of the technical aspect of Social Work. The applicant must have excellent professional judgement and must be willing to participate in training.
