Work Schedule:

Flexible schedule based on patient needs and facility requirements. Some weekend coverage may be required.

Role:

Perform comprehensive wound assessments, develop individualized treatment plans, and monitor healing progress. Provide advanced wound care treatments, including debridement, infection control, and dressing applications. Collaborate with facility nursing staff, caregivers, and primary care teams to optimize patient outcomes. Educate patients, families, and caregivers on wound prevention and care management. Maintain accurate and timely documentation of patient encounters. Travel between assigned SNFs, ALFs, and home visit locations as required. Stay current with best practices, treatment protocols, and advancements in wound care.

Requirements:

Active NP or MD license in Connecticut with prescriptive authority. Experience in wound care preferred, but training will be provided for motivated candidates. Ability to work independently in skilled nursing, assisted living, and home care settings. Strong communication and collaboration skills. Willingness to travel between multiple care locations.

Supplemental pay:

Bonus structure available based on productivity

Pay: $125,000.00 - $220,000.00 per year
